Why should you care about sleep apnea?
Most people think snoring is just a nuisance for the person sleeping next to them. But when snoring is caused by obstructive sleep apnea (OSA) — a condition where your throat repeatedly narrows or blocks while you sleep — it can quietly damage your health for years before you notice any obvious symptoms.
Studies show that in India, roughly 1 in 5 to 1 in 7 adults over 40 are at high risk for OSA, and it becomes more common with age and with extra weight around the neck and belly. Yet most people with OSA are never diagnosed — because the biggest effects happen while they are asleep.
What actually happens during an apnea event?
When your airway blocks during sleep, your oxygen level briefly drops and your brain has to partially wake you up to reopen the airway — often dozens or hundreds of times a night, without you remembering any of it. Each of these events:
- Sends a stress signal through your body, raising blood pressure and heart rate
- Triggers hormone and inflammation changes that make your body more resistant to insulin
- Disrupts the deep, restorative stages of sleep your body needs to regulate blood sugar and fat
Do this every night for years, and the effects add up — even in people who feel like they "sleep fine."
The four conditions most strongly linked to sleep apnea
- High blood pressure (hypertension)
OSA is one of the most common treatable causes of blood pressure that stays high despite medication, especially blood pressure that doesn't drop at night the way it should. If your BP is hard to control despite taking your tablets regularly, sleep apnea could be part of the reason - Poor cholesterol and triglycerides (dyslipidemia)
The oxygen drops during sleep apnea push your body to release fat into the bloodstream, which typically shows up as high triglycerides and low "good" HDL cholesterol — a pattern that raises heart disease risk even when your total cholesterol looks normal. - Type 2 diabetes and poor sugar control
Sleep apnea makes your body more resistant to insulin, even if you're not overweight. In people who already have diabetes, untreated sleep apnea is linked to harder-to-control sugars and a higher risk of complications affecting the eyes, kidneys, and nerves. - Metabolic syndrome (belly fat, high sugar, high BP, and abnormal lipids together)
Sleep apnea and metabolic syndrome feed into each other — extra weight around the neck and abdomen makes the airway more likely to collapse, and the oxygen drops from sleep apnea make it easier to gain weight and harder to control blood sugar and blood pressure. Treating one often helps the other.
Who should get screened?
Consider asking your doctor about a sleep apnea screening if you have:
- Loud snoring, or a bed partner who notices you stop breathing during sleep

- Morning headaches or a dry mouth on waking
- Daytime tiredness despite a full night in bed
- Blood pressure that stays high despite taking medication
- Type 2 diabetes or prediabetes, especially if sugars are hard to control
- A neck circumference on the larger side, or weight gain around the abdomen
What does screening and diagnosis involve?
A simple, quick questionnaire (such as STOP-Bang) done in the clinic can flag your risk in a few minutes. If your risk is high, the next step is a sleep study — either an overnight home sleep test or, in some cases, a supervised study — to measure how often your breathing is interrupted and how low your oxygen drops.
The good news: this is treatable
Sleep apnea is one of the few conditions where treating it can meaningfully improve blood pressure, blood sugar, and cholesterol all at once. Depending on severity, treatment may include:
- CPAP therapy (a small machine that keeps your airway open at night) — the most effective treatment for moderate-to-severe OSA
- Weight management, since even modest weight loss can reduce airway collapse
- Positional therapy and lifestyle changes for milder cases
- Coordinated management of blood pressure, sugar, and lipids alongside sleep treatment, since these conditions respond better when treated together rather than one at a time
