Real Patient, Real Results: When "Standard Treatment" Isn't Enough

A 58-year-old woman walked into our clinic with a familiar but frustrating story — type 2 diabetes and severe insulin resistance, BMI 32, and sugars that simply wouldn't budge despite being on a solid four-drug combination: metformin, pioglitazone, a DPP-4 inhibitor, and an SGLT-2 inhibitor.

To make things harder, she was gaining weight and her blood pressure was creeping up — two things that made her diabetes even tougher to manage, and left her feeling like she was doing everything right but getting nowhere.

We took a step back and rethought her regimen. We stopped the pioglitazone and DPP-4 inhibitor and started her on semaglutide.

The change over the next 4 weeks:

  • Lost 6 kgs
  • Blood sugars came close to normal range
  • Blood pressure trended down along with the weight
  • Mild, occasional gastric discomfort — the only side effect, and one she says is a small trade-off for how much better she feels

What made the real difference wasn't just "adding a new drug" — it was recognizing that insulin resistance, weight, and blood pressure are all connected, and treating them as one problem instead of three separate ones.

She's thrilled with her progress, and honestly, so are we. Stories like this are why we constantly reassess and personalize treatment instead of settling for "good enough" control.

Every patient responds differently, and treatment decisions should always be individualized by a qualified physician. This case is shared with the patient's consent to spread awareness about newer options in diabetes and weight management.

If you or someone you know is struggling with diabetes despite being on multiple medications, it may be time for a treatment review.
